Service Coordination (both initial and ongoing)
Little Meadows has been providing Early Intervention Service Coordination for families of children from Birth to 3 years of age, since 1994. We ensure that all children referred to us receive the rights, procedural safeguards, and services that are authorized to be provided under state and federal law.
Our Service Coordinators are your guide through the Early Intervention Program. They will help you with all aspects of the program, answering all of your questions as you go along the process. They provide assistance to families during the initial process of referral, evaluation, and if eligible, during the development and implementation of the Individual Family Service Plan (IFSP). Your service coordinator is the person who continually monitors your child’s services, making sure that all services are provided as directed. We employ both English and Spanish speaking service coordinators for the child and parent receiving Early Intervention services.
Our Early Intervention Program is funded and regulated by the New York State Department of Health and, (in New York City) by the New York City Department of Health and Mental Hygiene.
